Introduction:

Postoperative delirium is associated with increased morbidity and mortality, and should thus be avoided or recognized at an early stage. Although the pathophysiology of delirium is not yet fully understood, it is broadly accepted that the etiology involves multiple factors of different signifi - cance. It is known that an infl ammatory response develops following surgical procedures, and accumulating data suggest a role of the immune system in the pathogenesis of delirium.

Objectives: 

The aim of the current study was to investigate the association of infl ammatory biomarkers with the occurrence of postoperative delirium in elderly patients undergoing oncological surgery. 

Methods: 

Patients 65 years and older undergoing a surgical procedure for a solid tumour were prospectively included in this observational cohort study. The following infl ammatory cytokines were measured in plasma samples pre- and postoperatively at wound closure: C-reactive protein (CRP), interleukin-1 (IL-1), IL-6, IL-10, IL-12, and tumour necrosis factor-alpha (TNF-). The Delirium Observation Screening (DOS) scale was prospectively used for screening during the fi rst seven days following surgery. After discharge, medical records and the hospitals’ complication registration were screened for the development of postoperative delirium and the prescription of haloperidol, the standard anti-delirium drug in The Netherlands, during hospital admission after the fi rst seven days following surgery. Multivariate logistic regression analyses were performed.
